Job Description:
Experienced hospice or palliative care RN needed for hospice case management in Rutland /Addison county and surrounding area. Works closely with our interdisciplinary hospice team to provide Hospice care to patients/families in their homes, residential or nursing facilities. Requires taking share of on-call nursing coverage weekends and eves /holidays for hospice patients only, and reliable transportation. Work related mileage is reimbursed. We have a great team and are looking for highly skilled nurses who are passionate about providing the best possible care to our hospice patients!
